To apply Note 5 Theme you need go to CM Theme Chooser and select N5 Theme.
"Periodically System UI or Theme crashing" - It is doesnt means that app is unfinished or developer is silly and cant fix it.
The main reason of crashes are unstable / modified ROMs (experimental / nightly CM builds).
